What do I need to bring for my appointment?
Please bring your insurance card(s), any pertinent medical records, and a list of medications. Also, it is important to make sure you have the proper referral for your insurance, if it is required.
Do I have to pay at the time of my appointment?
You are responsible for any deductibles, co-insurance or co-pays your insurance requires. If we do not accept your insurance, you will be required to pay for your visit. If there is a balance on your account, our staff has been directed to collect the balance in addition to any deductibles or co-pays before your appointment.
Does the office validate parking?
Our Cedars Sinai office does not validate parking. Street parking is available on Burton Way and West 3rd St. Parking is also available at Beverly Center. Our Culver City office validates up to 30 minutes.
